In Mississippi, individuals aged 18 and older can obtain medical cannabis with a valid medical marijuana card.
Minors in Mississippi can access medical marijuana through a caregiver. The caregiver must be at least 21 years old, pass a background check, and obtain an identification card to purchase and administer cannabis.
This ensures regulated access for minors who need medical marijuana for qualifying conditions.
*Please note for patients under 24 years old, two evaluations are required to certify for a MC card.

Visit a certified medical practitioner to get a recommendation for certification based on your diagnosis. Then, apply through the MMCP's online portal. There are self pay costs affiliated with obtaining your card at our clinic. Expect a fee to obtain your card through the department of health once certified through the MDH.

The following medical conditions or their treatment qualify for participation in the Mississippi Medical Cannabis Program:
cancer
Parkinson's disease
Huntington's disease
muscular dystrophy
glaucoma
spastic quadriplegia
positive status for human immunodeficiency virus (HIV)
acquired immune deficiency syndrome (AIDS)
hepatitis
amyotrophic lateral sclerosis (ALS)
Crohn's disease
ulcerative colitis
sickle-cell anemia
Alzheimer's disease
agitation of dementia
post-traumatic stress disorder (PTSD)
autism
pain refractory to appropriate opioid management
diabetic/peripheral neuropathy
spinal cord disease or severe injury
Also qualifying is a chronic terminal or debilitating disease or medical condition or its treatment that produces one or more of the following:
cachexia or wasting syndrome
chronic pain
severe or intractable nausea
seizures
severe and persistent muscle spasms including, but not limited to, those characteristic of multiple sclerosis

Patients must be registered and have received their card from the MMCP. received his or hers medical card from the Mississippi Medical Cannabis Program (MMCP)
Then, patients must complete and submit the caregiver form below and submit it:

A fingerprint background check is required by the MSDH. The background check includes the fingerprints. The background check is done by MMCP after your fingerprint card has been submitted along with the caregiver application and affidavit.

This includes your initial appointment and consultation with one of our certified providers.

*Both fees are for patients who have completed the initial MC certification at AGMT.

We offer online assistance with completion of the MDH application through their website in office for a small fee of 15.00. You must pay associated fees when choosing this option that MDH has established for MC cards and is not affiliated with AGMT payment processes.

Gather Information
Gather information and documents that are required for your appointment. To schedule, you will need a working email address that you check often. You will use this email at our clinic and on the MDH website. It MUST be the same email.
Ensure your Mississippi ID is current, has you current address, correct date of birth, spelling of your name, and will not conflict with the information we require for certifications. You must have documentation that details a prior diagnosis by a licensed medical professional to upload to your profile before your appointment.

Book Your Appointment
Once your information is collected, you will need to schedule your appointment online. Payment for associated fees is required at the time of scheduling and there are no exceptions. Please note that these fees are nonrefundable.
We stress that it is important to upload your paperwork; failure to do so will result in the cancellation of your appointment.
