Schedule

To make, change, request, or schedule an appointment with your provider, please use the AdvancedMD patient portal to do this. This tab is found on our homepage, but if you can’t find it, call us and we can help.

Please send us your name, address, preferred phone number, email, insurance carrier, insurance ID number, and appointment preference time, and physician preference (if any) to referralsagmt@agoodmindtoo.com.

Please note if you do not include all of this information, we cannot schedule you in the clinic until we receive it.

Completely fill out the appropriate forms under the tab found on our homepage prior to your scheduled appointment to reduce processing times when you arrive. We just want you to be prepared for success!

First of all, we want to extend a sincere “Thank you” for trusting us to provide mental health care to your patient. Our referral form can be found here or on our homepage as well.

Please read the instructions carefully so that we can expedite and schedule your patient as soon as possible. We don’t want you to miss anything that can help your patient to be seen in our office. Once this is complete, you may fax the information to 6015332566.
